ASSISTANT DIRECTOR OF NURSING 

We are currently looking for a dynamic team member that will assist the Director of Nursing to ensure high standards of patient care.  You will assist in planning, organizing and directing the day-to-day nursing department.  You will have the opportunity to ensures quality care that is consistent with the facility’s and industry’s regulatory standards.  You will be able to impact the lives of each resident and patient in our community by being a critical part of the management team and overseeing the overall direction, coordination and evaluation of nursing care and services provided to the residents.  
 Requirements
• Ensures nursing staff is providing quality and appropriate resident / patient care that meets or exceeds company and regulatory standards. 

• Monitors monthly performance of nursing services in relation to budget and intervenes as needed. 

• Schedules and performs resident / patient rounds to monitor and evaluate the quality and appropriateness of nursing care. 

• Maintains administrative authority, responsibility and accountability for the proper charting and documentation of care and of medications and treatments.

• Leads and manages the wound program to ensure that adequate treatments and interventions are implemented. 

• Knowledgeable of general, rehabilitative and restorative nursing procedures, laws and regulations and guidelines governing long-term care

• Recruits, hires, provides orientation/training, and retains a sufficient number of qualified staff to carry out nursing programs and services.  Ensures employee performance meets or exceeds expectations and periodic performance appraisals are conducted on a timely basis. 

• Develops and implements the written staffing plan and nursing schedule that reflects the needs of the resident and resident population. 

• Ensures all required records are maintained and submitted, as appropriate, in an accurate and timely manner. 

• Evaluates current and potential residents and patients to ensure facility can provide appropriate level of care.  

• Maintains regular and ongoing communication with facility Medical Director ensuring programs and care services meet the needs of patients / residents. 

• Completes required forms and documents in accordance with company policy and state and/or federal regulations. 
Manages clinical aspects of state or federal government survey processes. 
Job Requirements
* RN or LPN license in good standing with the state 
* Previous long-term care experience 
* Previous supervisory and leadership experience
* Excellent communication, clinical and mentoring skills
* Experience in quality assurance and advanced training in infection control procedures 
* Caring bedside manner and positive attitude toward the patients and residents and the staff
* Desire to learn new skills and ability to use an electronic medical record
* Ability to multi-task in a fast-paced environment
* Data entry skills, experience with Microsoft programs and Matrix electronic charting a plus

  • Make beds, clean bedside and overbed tables.
  • Dress residents neatly and in their own clothing. Assist residents in dressing as necessary.
  • Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.).
  • Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities.
  • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary.
  • Ensures that all consumer/resident rights are protected.
  • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator.

4.                  Represents the Patient Care Services departments in organizational and strategic planning.  Develops and implements action plans in order to meet established MHC and Corporate goals and priorities.
5.                  Develops a plan in collaboration with MHC’s Long Term Care Administrator and the patient care team for the provision of patient care services consistent with the organizational strategic plan, goals and objectives.
6.                  Provides managerial and leadership development to Clinical Nurse Managers and direct reports.  Motivates, mentors and establishes accountability expectations for quality nursing unit management and care outcomes.
7.                  Supports customer and service excellence initiatives throughout the nursing department, setting high standards for successful resident and patient satisfaction survey outcomes.
8.                  Develops, reviews, submits and manages departmental operating and capital budgets prepared by direct reports.  Ensures budget performance and variance accountabilities.
9.                  Provides leadership, direction and consultation in human resource management.  Supports and initiates nursing recruitment and retention strategies in collaboration with the Human Resources department.
10.              Provides a formal liaison between patient care services, senior management, department managers and medical staff.  Promotes effective communications among all customers including residents/patients, families, physicians, other Masonicare affiliates, board members and the community.
11.              Provides consultation and guidance to direct reports and assigned staff.  Coordinates with Supervisor of Nursing Education for Nursing Education planning and the implementation of educational activities for nursing and other patient care services staff.
12.              Establishes and supports an environment conducive to nursing and other patient care related clinical research.
13.              Attends meetings and prepares reports as required.  Serves on various MHC and Masonicare committees and participates in professional growth by attending approves seminars, workshops, and in-service programs.

Minimum Qualifications:

Education:
BSN required.

RN Licensed in State of Connecticut with 1 year of additional education or experience in rehabilitation or geriatric nursing and one year of nursing service administrator.

Experience:      
5-7 years progressive experience in nursing administration which includes a minimum of three (3) years in a senior management and leadership position as a Director of Nursing or Vice President for Patient Care Services.  Previous LTC experience is preferred along with a thorough up-to-date knowledge of LTC codes, regulations, MDS and reimbursement issues.

Certificates, Licenses, Registrations:
                       Registered Nurse in the State of Connecticut or eligible to be licensed.

Represents the unit and is a liaison to internal and external customers. Participates in professional development activities and is active on hospital-wide committees. Demonstrates various styles of management. Participates in unit(s) budget development, responsible for monitoring and variance analysis.

The Director of Nursing (DON) shall be a Registered Nurse who is responsible for the overall direction of home health clinical services.  The DON will have overall responsibility for the quality, adequacy of services provided and supervision of staff.  The DON establishes, implements and evaluates goals and objectives for home health services that meet and promote the standards of quality.  Business focus is on skilled homecare for worker's compensation insurance (wound care, IVs, assessments) and private duty homecare for the elderly (no Medicare, Medicaid or OASIS documentation).

  1. Provides care as defined within the scope of Maryland's nurse practice act.

  3. Initiates the plan of care and makes necessary revisions.
  4. Assigns nurses to skilled care cases (RNs, LPNs)
  5. Supervises or may on occasion provide  those services requiring substantial and specialized nursing skill (wound care, IV therapy, trach and vent care and others.)
  6. Reviews and may write nursing procedures.
  7. Reviews and may prepare clinical and progress notes and summaries of care.
  8. Coordinates with workers compensation case managers and catastrophic care managers.
  9. Obtains doctors orders and signatures on plan of care, based on in-home assessment.
  10. Informs the physician and other personnel of changes in the patient’s condition and needs.
  11. Counsels the client and family in meeting nursing and related needs.
  12. Assists in the development of organization goals.  Develops, recommends, and administers agency policies and procedures.
  13. Hires RNs and LPNs and manages their scheduled and competency assessments.
  14. Assesses competency of non-skilled staff and ensures staff is assigned in a manner that promotes quality, continuity and safety of a client’s care.
  15. Supervises, educates and coaches RNs, LPNs and CNAs.
  16. Plans and implements in-service and training programs.
  17. Conducts performance evaluation of medical care-giving staff.
  18. Oversees the maintenance of client clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities.
  19. Assures proper maintenance of clinical records in compliance with local, state and federal laws.
  20. Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel.
  21. Identifies and participates in performance improvement activities.
  22. Stays informed about changes in the field of nursing and home health care.  Shares information with appropriate organization personnel.
  23. Promotes home health referrals in the health care community and assists sales and marketing as requested.
  24. Shares off- hours phone coverage nights and weekends

EDUCATION and/or EXPERIENCE

  3. Minimum five (5) years of clinical experience with at least half of that time working in home health or related health care organization.  Minimum three to five years of direct employee supervision.
  4. Demonstrated ability to supervise and direct staff.
  5. Possess exceptional nursing assessment skills
  6. Comply with accepted professional standards of practice.
  7. Proven ability to build quality and implement improvements within the operation.
  8. Excellent interpersonal and communication skills.
  9. Analytic/Critical thinker.

Serving as the key leader of the interdependent team, the RN is responsible to achieve patient and unit outcomes. The RN coordinates the care for a group of patients. Including, but not limited to, comprehensive assessment of patient and family needs; identification of outcomes based on the needs assessment; establishing the plan of care; monitoring the progress; and modification of the plan of care as needed

Demonstrates commitment to the universal guiding principles of the patient care model in order to achieve the HealthEast Nursing Vision.

Utilizes the Nursing Process to coordinate and achieve outcome focused patient care.

Communicates effectively with patient/family, physicians, the interdependent team and other disciplines regarding plan of care.

Identifies and documents learning needs of the patient and family and provides individualized education.

Complies with regulatory requirements, patient safety requirements, infection control practices, and other established practice standards, policies and procedures.

Participates in the development and achievement of unit goals (Safety, cost, quality, satisfaction, engagement).

Coordinates the interdependent team while promoting collaborative teamwork and cooperation.

Achieves customer service excellence by understanding the needs of all customers i.e. patients, families, and physicians.

Maintains confidentiality in all aspects of the job.

Performs all other related job duties as assigned.

Variable work schedule, must be able to work every Monday, Tuesday, Wednesday, Thursday, Friday, Saturday and Sundays. Future weekend rotation may change depending on unit need.      

Qualifications:

Education:
Graduate of accredited School of Nursing. BSN/BAN preferred. Current license to practice registered nursing in Minnesota. Current Basic Life Support certification.

Experience:
One year experience preferred.

Special Knowledge, Skills, and Abilities:
Ability to utilize the nursing process and critical thinking to provide evidence based patient care;
Ability to coordinate care for a group of patients;
Ability to lead the interdependent care team;
Ability to effectively document and communicate all aspects of the nursing process;

JOB SUMMARY

The Registered Nurse performs and documents assessments; ensures health history is completed; provides patient care which is appropriate to the patient diagnosis, symptoms and physician orders; identifies significant changes or high-risk situations in patient’s condition; completes all documentation; communicates patient progress; ensures completion of post discharge; assists with coordination and preparation of transfer, when necessary and participates in measuring and monitoring quality indicators and outcomes.  Patients cared for include infants, children, adolescent, adult and geriatric.



Required:

•           Registered Nurse.
•           Licensed by the North Dakota Board of Nursing (if a resident of North Dakota or changing residence for longer than 30 days) or licensed by a member of the “Compact State Agreement” if not a North Dakota resident.  (If changing residence from a Compact State to North Dakota, will need to obtain a North Dakota license within 30 working days of obtaining new residency.)
•           Nurses who provide nursing care to patients calling from Minnesota will need to be licensed by the Minnesota Board of Nursing or will obtain their Minnesota license within eight (8) weeks from their first day of work (telephone calls from patients calling from Minnesota can not be taken from any nurse unless they have obtained their Minnesota license).
•           Successful completion of the functional assessment.

Preferred:
•           Past experience with Special Procedures, Surgery or GI Endoscopy.
•           BS in nursing.
•           One (1) year of experience in a hospital or clinic setting.
